Dear Dr. Roberts, I am a 42 yr. old female on peritoneal dialysis which I do
daily at home. I am on dialysis due to renal failure caused by Lupus Nephritis
(SLE). I have no hypertension, no cardiac problems, no hearing loss, no inner
ear/balance problems, no mental/emotional problems and the SLE is not active. I
take no medications that alter any of my faculties that could impair my ability
to pilot aircraft. I have been trying, unsuccessfully, to find out whether I
would be able to obtain medical certification for a rating above private pilot
(i.e., non-airline commercial). Please advise. Lenora

Your case is complex. You will need to apply for a special issuance, which
entails you submitting all of your medical records, and a current comprehensive
medical report from your treating nephrologist.

Dear Dr. Roberts, I am a 38 years old male, with a Class III, and have a life of
excellent health. I have been seeing the same FAA Medical Examiner as my (late)
Father since receiving my student pilot license 19 years ago. I, as did my
father, have always showed trace amounts of blood in my urine (hemoglobinuria).
My examiner had always told me this was hereditary and did not report this to
the FAA. Due to my Medical Examiner's retirement, I recently had to see a new
Medical Examiner, received a Class III and subsequently received a letter from
the FAA regarding this condition (and requesting more information). My personal
physician recommended an IVP which was negative and completely within limits.
She wrote a letter to the FAA, and I have my medical for 2 more years. I
don't know how to 'treat' a condition that I've had for 19 years, but I also do
not want to have to submit to IVPs every 2 years for the rest of my private
flying career. I would very much appreciate any help or information you could
offer about hemoglobinuria. Thank you very much. Paul

Congenital hemoglobinuria is a benign condition, and often seen in an AME setting.
Just bring in a letter from your doctor whenever your medical is due indicating
that you are asymptomatic and the condition is hereditary.

Would there be any problem with a Class II or III physical for
a person who had a kidney removed? There are no other problems - the remaining
kidney is working normally and there are no other complications.

As long as the remaining kidney is giving you normal kidney function,
there should be no problem. But, you will have to apply for a special
issuance. You will need a letter from your Urologist along with BUN and
Creatinine blood test results, and a urinanalysis.

If you had only one episode that was remote, no tests are needed.
